*1. Physical examination record for foreigner
Please take it to hospital to have a checkup with all items, including personal information, photo, checking date, doctors' suggestion and signature.
2. Acceptance letter of supervisor
3. Table of contents and abstracts of your academic publications and any other original achievement in your field
*4. Photocopy of valid passport
With name, passport number & expiration date, and photo included
*5. Passport photo
A recent passport-sized photo of the applicant
*6. Academic Transcripts of bachelor and master degree
Academic Transcripts in languages other than Chinese or English should be translated into Chinese or English and be certified by notarization. Both original and official English translated version are required.

*8. Two letters of recommendation
Written in English from two respectable professors. including professors' contact no, email address and handwritten signature.
*9. Personal Statement
Above 1500 words for doctorates, must be in Chinese or English;

Provided by your current/previous school/company in Chinese/English to improve your success rate.
